PLUS Payments:

When adding a client, that is already installed, to "Auto Pay" you must verify the client is current on their payment by looking at the "Paid to Date" (not the "Service Date"). If client is not currect, you must collect the past due payment and a 30/60 day payment going forward or a device will not be mailed out.
If a client re enrolls back onto autopay upon approaching their service date, there should be a UTR/CNG ticket submitted.
Make sure the client’s account is in IA -Mail, if not please go to “Update Client Info” and change the location to “Mail” then save.
Please verify the shipping address with client, does the client have an apartment number and is the street spelled correctly. If the address is different in PLUS, ask if this is a new address, if so update address in PLUS.
Verify the “Next Service Date”, if the client is about to lock out (past due) set him/her an appointment at one of our full service shops instead or offer overnight/Saturday delivery. (If client is going to a location for service make sure the file is in the correct location NOT IN MAILER). This would be at the clients discrection, driving distance needs to be addressed with client.
If client is past due and locked out then a $25.00 Late Payment fee needs to be added (Use “Late Fee” from the Fees drop down.).

For Monitors select from “Add Fees”(After selecting each fee needed click “Add”)

Daily Monitoring FC100 (Charged per day)
Reporting Fees (Charged per day, needs to match the number of monitor days)
Daily Loss Protection Plan (If they have LPP, added the number that matchs monitor days)
If they are IA ordered and paying for less than 60 days charge the $10.00 “Early Service Fee”.
If client is past due and locked out then a $25.00 Late Payment fee needs to be added (Use “Late Payment Fee” from the Fees drop down.). This is NOT a missed appt fee.

*If the client is pro-rating days to their removal date, please waive the $10 Early Service Fee*

Make sure you document your payment in the NOTE section of the invoice and to the UTR ticket so we know the brakedown of what was paid for.

Example: “Client made a payment 60 days, $139.10 auth code 12345-your name, ship to same address on file, please send extra mouthpieces…MM”

SHIPPING INFO: The Grimes office ships the devices to the clients, they are open M-F 8am-5pm. Shipments do not get sent out on the weekend or after 2 pm during the week. Please keep this in mind when telling a client when they will receive an expedited request.

Devices are not guaranteed to ship same day and may take place within one or two business days of payment. We will still honor same day expedited shipping as long as client pays the additional charge and shipping department is properly notified (2pm is cutoff time for UPS shipping). UPS does NOT deliver to a PO BOX.
Once device is shipped USPS, in Iowa, it will take 2-3 business days to receive device, Holidays could take longer.
If client pays for expedited shipping, please mark the ticket "Urgent". Do NOT make a ticket "Urgent" if expedited shipping is not paid. Shipping fees need to be collected even if the client is on Auto Pay.
Shipping is included in monitoring fee for all 60 day monitors and all reset appointments.
The only time a client pays for shipping is when they need it expedited, or sent via UPS. UPS fees shown below:
UPS Ground: $25.00
UPS Next Day: $65.00 (IF IT IS FRIDAY BEFORE 2PM CST, THEY MUST PAY THE SATURDAY DELIVERY CHARGE. IF THE $65 IS NOT PAID THE DEVICE WILL SHIP ON MONDAY)
UPS Saturday Delivery: $75.00
All Shipments are shipped Postal Service (USPS) unless the client pays for UPS shipping - UPS does NOT deliver to a PO BOX
Always ask the client if they need extra mouthpieces.
